9. Healthcare & Non-Clinical Services Disclaimer

Patient Care Health is primarily an administrative and management services organization.

Unless expressly stated otherwise in a specific service arrangement, PCH’s services are non-clinical and do not replace medical care provided by appropriately licensed healthcare professionals.

PCH’s Remote Patient Monitoring administrative services may include eligibility verification, device distribution coordination, patient onboarding support, connectivity oversight, dashboard administration, identification of data transmission gaps, and administrative billing support.

PCH does not provide clinical monitoring, clinical interpretation of patient data, medical triage, or patient care as part of its standard RPM administrative services.

PCH’s Chronic Care Management administrative services may include eligibility list maintenance, enrollment and consent coordination, administrative documentation, time tracking, care plan document maintenance based on information provided by the provider, and billing-support documentation.

PCH does not provide clinical care-plan creation or modification, clinical assessments or interventions, determinations of medical necessity, or clinical patient outreach as part of its standard CCM administrative services.

Licensed healthcare providers remain responsible for clinical judgment, diagnosis, treatment, patient care, medical necessity determinations, and other clinical responsibilities.

10. Operational Compliance Disclaimer

Patient Care Health may provide administrative compliance and regulatory support related to payer operations, documentation requirements, network participation, roster accuracy, administrative audits, and related operational matters.

PCH does not provide legal advice and does not replace legal counsel or other qualified professional advisors.

Compliance support does not constitute a guarantee of regulatory compliance, payer acceptance, audit results, network participation, reimbursement, or other outcomes.

Organizations remain responsible for understanding and satisfying the legal, regulatory, contractual, clinical, coding, and other requirements applicable to their operations.

11. Credentialing & Enrollment Disclaimer

Patient Care Health may provide administrative preparation, submission, tracking, and support for payer credentialing and network enrollment.

Services may include collection of provider documentation, preparation of payer-specific applications, CAQH profile maintenance, submission tracking, status monitoring, follow-up, and provider-directory management.

PCH does not control payer credentialing or network participation decisions and does not guarantee approval, enrollment, effective dates, reimbursement arrangements, or acceptance into any payer network.

12. Communications Consent

When you voluntarily provide your contact information and affirmatively consent where required, you authorize Patient Care Health to communicate with you regarding your inquiry, requested services, programs, solutions, appointments, support requests, educational information, operational information, and other communications you have requested or agreed to receive.

Where permitted by law and based on the consent provided, communications may use automated technology, automated dialing systems, prerecorded or artificial voice messages, or automated text messaging systems.
